

GROUP EMDR APPLICATION & REGISTRATION

We’re glad you’re interested in joining our EMDR group. This form helps us make sure the group is the right fit for you and gives us the information we need to support your participation. Everything you share is confidential.
Once you’ve submitted your application, one of the facilitators (Erin or Josette) will be in touch. We’ll arrange a short phone call with you about two weeks before the group begins to answer questions, give you more details, and confirm readiness.
Important: Your place in the group is not secured until you’ve received a confirmation email and your payment for the three sessions has been made.
Some information from this form may be used (anonymously) to help us refine and improve our groups.
Please note:

Participants need to have a therapist, HIP, GP, or another health professional they can connect with if extra help is needed between the sessions.

Currently, this EMDR Group is not available as adjunct therapy to ACC sensitive claims or other ACC-funded treatment.

We welcome participants of all genders and gender identities. If you would prefer a group for female-identifying or non-binary participants only, please let us know — we’re gauging interest for future groups.
Fees & Payment

One round of Group EMDR = 2 sessions (first session is 120 minutes, second session is 90 minutes)

Total fee: $395 (inc GST) for the two sessions

Full payment is required two weeks before the group starts to secure your place

Spaces are limited
Refund & Withdrawal

Withdrawals made within the final week before the group, or after the group has started, are non-refundable (as we can’t refill places at short notice).

If, during the first session, we assess that you’d benefit from more stabilisation before continuing, you’ll receive a refund for the remaining session ($150 inc GST). You’re welcome to join a future round when ready.

In rare or exceptional circumstances, facilitators may use discretion to offer refunds outside these guidelines.

Groups require a minimum number of participants. If this number isn’t reached, we may reschedule or cancel. If rescheduled dates don’t suit you, you can transfer to another group or a full refund will be issued if you prefer that.

If unforeseen circumstances require us to cancel a group entirely, full refunds will be issued.
